When an
offender is sentenced to a term of incarceration at NDOC, the victim may fill
out a Victim Notification Request from available from the agency which
prosecuted the case and following submission instructions on the form.
To request registration as
a victim after the prosecution’s case is closed, or update your contact
information for an existing registration, submit a written request to the VSU including
the offender’s full name and NDOC Offender Identification Number in the subject
line of an email or physical letter.
All requests require the
following Please include the following information:

Your full name and relationship to the victim or
offender

The reason for your request
·
The name of the county in which the offender was prosecuted
·
Your current physical mailing address, email and phone
number
·
If you are updating your registration, you must also
include the physical mailing address you initially or most recently used
to register
